Entry Details
To participate, individuals can fill out the online entry form available on the campaign website. Only a single entry is permitted per person throughout the promotion period. The sweepstakes is open to individuals living in the fifty United States and the District of Columbia who are at least 18 years of age. All entries must be received before the deadline on October 19, 2026.
The giveaway features multiple prize tiers. One selected first prize winner will receive an Aflac prize pack. Additionally, 75 winners will receive a limited-edition Fit Checks jacket by Rachel Zoe, with 26 winners receiving a large/extra-large size jacket and 49 winners receiving a small/medium size jacket.
